Before your next appointment
6 steps, in order.
- Read the doctor scripts and pick the 1 or 2 you might need.
- Track your symptoms for at least 2 weeks on the symptom tracker.
- Fill in Patient Passport Part 1, your history on one sheet.
- Write your questions on Part 2, one per line, before the visit.
- Print both parts from the Toolkit page.
- Hand Part 1 to the nurse at check-in, before you even see the doctor.
